The appearance of a fixed and dilated pupil alongside iris atrophy, following corneal graft in a small group of keratoconus patients who receive atropine (mydriatics). The wide and rigid pupils may remain so for weeks or even months. It can occur in association with other surgical procedures such as glaucoma surgery, cataract surgery, posterior synechiae, lamellar keratoplasty, ectropion uvea, etc.
